This procedure was several years ago and we now have a few key modifications. First, we no longer graft into the muscle as there have been safety concerns. Second, we use PAL while grafting to achieve better fat distribution and stiffer, safer cannulas. Third, most of the fat is not grafted from below as shown in the video. Fourth, we typically graft more volume than demonstrated in this video as this was a conservative procedure. Fourth, we have a closed fat system where the fat is not grafted through a pump system rather than transfer to syringes. Live plastic surgery video.

This video was nearly a decade ago and was performed a little differently than we may now. The key elements of the procedure are the same but a few advancements have been made.
1) We use cautery to divide the muscle. Not a major difference in technique but we have found a small decrease in postoperative pain
2) We soak our implants in a betadine solution
3) We use barbed, knotless sutures for closure

Arm lift (Brachioplasty) plastic surgery, in the office of Utah plastic surgeon Dr. York Yates. Arm lift is a common procedure after weight loss surgery. The procedure is done with local anesthesia and sedation. Real surgery video. The patient has no pain but the video is graphic and not for the weak of stomach.
This is an older surgical video and we generally prefer a deeper level of sedation now. It is obviously possible to do the procedure with lighter sedation, it is just our preference. There are a few other modifications in the procedure but for the most part this has withstood the test of time. 2020 Update: This procedure is several years old now and there are a few modifications we should point out:

1) We now prefer to put the implant in first before committing to the skin removal
2) The "pedicle" is more often superior (technical issue)
3) Suture techniques are more commonly barbed 'knotless' sutures
